Permission Form

In accordance with the Enrolment Application Process, permission is given by the Legal Guardian/s of a potential student to allow the Principal or College Representative to contact and record any relevant information (either verbally or via documentary material or records) about the applicant.

(Legal Guardian) hereby authorise St Augustine's College (Principal or College Representative) to collect information (either verbally or via documentary material or records) from the following existing and/or previous school/s.

I/We understand and acknowledge that the information accessed will be shared and stored by the Catholic Education Diocese of Cairns organisation strictly for the purpose of our son's enrolment application and ongoing education provisions. I/We, understand that failure by the existing school/previous school to comply with any request for further information may result in delaying our son's application and/or Offer of Enrolment with St Augustine's College.
